This is a standard form but the form submission will be sent to various emails (hardcoded - set by developers) depending on what has been selected from the "Enquiry type" dropdown.

For example "General enquiries" form submissions will go to email 1 but "Make a complaint" submissions will go to email 2.

By default, the form submissions are not stored in the database unless the option "Store submitted results of form" is checked.
